Secondary hyperlipidaemia.

چکیده

The secondary hyperlipidaemias occur remarkably frequently, and the underlying cause of high serum lipid levels will often be missed if it is not actively sought. Diagnosis is important because the lipid abnormality responds to treatment of the primary disorder in most cases; notable exceptions to this, however, are the hyperlipidaemias associated with gout and chronic renal failure. The conditions regularly associated with hyperlipidaemias are listed in the table. There is no lipoprotein pattern which is specific for any one of these, and almost any pattern may occur in every condition; the pattern may even vary from time to time in the same patient.

A case of membranous glomerulonephritis presenting as pulmonary embolism and acute hyperlipidaemia.

This paper describes the case of a 48 year old man who presented with acute hyperlipidaemia following pulmonary embolism. Subsequent investigation revealed that the hyperlipidaemia was secondary to nephrotic syndrome of glomerulonephritis. The case illustrates the importance of investigating acute hyperlipidaemia for its underlying causes.

The erroneous haemoglobin-hyperlipidaemia relationship.

Hyperlipidaemia, whether primary or secondary to parenteral feeding with intravenous lipid emulsions, causes the haemoglobin level to be erroneously high when measured spectrophotometrically. Contrary to a previous report, evidence is presented which suggests a relationship between the amount of the false rise in the haemoglobin level and the type and degree of hyperlipidaemia.

Lessons from treatment resistant hyperlipidaemia

A 68-year-old woman was referred to Lipid Clinic with sudden deterioration of previously well-controlled primary hyperlipidaemia. Investigations revealed nephrotic range proteinuria, leading to urgent renal biopsy and a diagnosis of amyloidosis. Chemotherapy was successful in stabilising renal function, reducing proteinuria and eliminating serum paraprotein. The resistant hyperlipidaemia subseq...

Misleading plasma electrolytes in diabetic children with severe hyperlipidaemia.

Severe abnormalities of plasma electrolyte concentrations were observed in 4 children who had massive hyperlipidaemia secondary to poorly-controlled, insulin-dependent diabetes mellitus. These electrolyte values were fallacious and invoked problems of clinical management, particularly with regard to fluid replacement. Each case is described and the interpretation of plasma elecytolyte values in...

Management of patients with nephrotic syndrome.

Nephrotic syndrome is characterised by proteinuria >3.5 g/24h, oedema, hypoalbuminaemia and hyperlipidaemia. Several glomerular diseases, either primary or secondary, may lead to nephrotic syndrome. Investigations for nephrotic syndrome include immunological and infectious evaluations. Renal biopsy is often mandatory, except in diabetes. Depending on aetiology specific treatment, often with imm...

Sheehan's syndrome presenting with type III hyperlipoproteinaemia.

A patient is described in whom the presenting feature of hypopituitarism was the development of palmar xanthomata associated with type III hyperlipoproteinaemia. Treatment of her secondary hypothyroidism with thyroxine caused resolution of the xanthomata and hyperlipidaemia, but the underlying compositional abnormality of the lipoproteins could still be observed.
